Stamped statement letter that the applicant “is not a person in a Public Accounting Firm, Legal Consultant Firm, Public Appraisal Service Office or other party providing insurance services, non-insurance services, appraisal services or other consulting services to Bank Jatim within 6 (six) ) last month” (CODE: KU)
5. Stamped statement letter that the applicant "is not a person who works or has the authority and responsibility to plan, lead, control, or supervise the activities of Bank Jatim within the last 6 (six) months, except for independent commissioners" (CODE: KU & KR)
6. Stamped statement letter that the applicant "does not own shares directly or indirectly in the issuer or public company (Bank Jatim)" (CODE: KU)
7. Stamped statement letter that the applicant "does not have any affiliation with members of the board of commissioners, members of the board of directors, or the main shareholder of the issuer or public company (Bank Jatim)" (CODE: KU)
8. Stamped statement letter that the applicant "does not have a direct or indirect relationship with the business activities of Bank Jatim" (CODE: KU & KR)
9. Stamped statement letter that the applicant is "Willing to continuously improve competence through education and training" (KODE: KU)
